1) Call 904-679-3233 to schedule appointment time
2) Fill out our online New Patient Intake Form. Visit here
– additional forms may be emailed TO YOU
– x-rays and records may be requested FROM YOU
3) Both new and established patients, fill out SRS22 Questionnaire. Visit here
4) Make payment for TeleHealth visit 24 hours prior to visit. Visit here
5) An online link for the TeleHealth appointment will be sent to you via email or text
6) Make sure your computer, tablet, or cell phone has a microphone and camera for the appointment. The visit will be a face-to-face encounter with Dr. J Hartley
7) Log on to the link at appointment time
